BRIEF REVIEWS The Relationship among Cardiac Pacemakers Overdrive Suppression
نویسنده
چکیده
THE RELATIONSHIP among cardiac pacemakers is characterized by the fact that usually one pacemaker is dominant and all the others are subsidiary. The sinoatrial node acts as the dominant pacemaker and all other pacemaker tissues are discharged by a conducted impulse before their diastolic depolarization can attain threshold. These pacemakers are called subsidiary to emphasize the fact that under normal circumstances they are engaged in conducting impulses but under abnormal circumstances they may become actual pacemakers. Subsidiary in this sense means that one or the other of these pacemaker tissues assumes the function of impulse formation when the dominant pacemaker fails to initiate the activation of the heart. On the basis of these recognized facts, a dominant pacemaker can be defined as the pacemaker tissue which first succeeds in attaining threshold and in initiating a propagated impulse. The other pacemakers are subsidiary because their diastolic depolarization fails to attain the threshold before a propagated impulse initiates local excitation. Although these concepts are undisputed, certain observations seem to be at variance with the idea that the relationship among pacemakers involves only a successful race to the threshold. This is clearly shown by the fact that a sudden suppression of a dominant pacemaker is followed by a period of quiescence before a subsidiary pacemaker begins to discharge rhythmically. The period of quiescence is far longer than the interval between two beats of the newly established subsidiary rhythm. This suggests that subsidiary pacemakers are somehow kept suppressed. This form of inhibition will be referred to as overdrive suppression.

متن کاملThe relationship between overdrive suppression and overdrive excitation in ventricular pacemakers in dogs.
We studied the excitatory and inhibitory effects of overdrive on idioventricular pacemakers in anesthetized dogs with recently induced complete atrioventricular block. The following results were obtained: (1) a slow driving rate may induce a temporary rhythm which may be reinstituted with additional stimuli; (2) the induced rhythm may appear as coupled extrasystoles which, on interruption of th...
متن کاملHeart Block After His-Bundle Ablation
To investigate the electrophysiological properties of ventricular impulse formation after His-bundle ablation in 11 patients, incremental ventricular overdrive stimulation studies were performed. The studies, which were spread over a follow-up period of up to 601 days, were carried out invasively with temporary leads as well as noninvasively with the implanted pacemakers and chest wall inhibiti...
متن کاملOn the mechanism of idioventricular pacemaker suppression by fast drive.
The role of vagal activity, potassium concentration, and temperature in overdrive suppression of ventricular pacemakers, was studied in anesthetized dogs with and without surgical atrioventricular block. The results were as follows. (1) Overdrive of an idioventricular pacemaker during vagal stimulation was followed by a second pause. (2) Hypothermia increased the duration of the vagal and overd...
متن کاملEfficacy of the DDD+ Overdrive Algorithm for the Suppression of Atrial Tachyarrhythmias
Atrial fibrillation (AF) persists as the most challenging arrhythmia in clinical practice, and it currently affects an estimated 400 million people worldwide. We view AF as the most common sustained supraventricular tachyarrhythmia, for it is associated with extensive morbidity and mortality [1]. According to some studies, AF is responsible for numerous hospitalizations, which result in signifi...
